Family Model Program
A Popular Alternative to Residential Facilities: Our Family Model Program
The Family Model Program at Mid-South Care Services redefines full-time residential care by providing private single-family homes as full-time residences for seniors and adults with physical, intellectual and developmental disabilities throughout the West Tennessee Region (west of the Tennessee River). In this fast-growing program, a host family becomes the primary caregiver for up to 3 adults in need who choose to live with that family instead of living in a facility. These compassionate families open their warm homes to provide full-time care including room and board. It’s an approach that goes beyond providing a place to live; it’s about fostering a sense of belonging, promoting emotional well-being, and creating a supportive community where seniors and adults with physical, intellectual and developmental disabilities can reside with grace and dignity.
Our Family Model Providers
Our Family Model Providers are caring and compassionate people who open their homes to become the primary caregivers and provide full-time room and board for up to 3 seniors or adults with physical, intellectual and developmental disabilities.
Our Family Model Clients
Our family model clients are seniors and adults with physical, intellectual and developmental disabilities who aren’t able to live by themselves but would prefer to live with a host family in a private home instead of living in a facility.
Matching Clients and Family Model Providers
Mid-South Care Services utilizes a personal matching system, considering personalities, lifestyles and care requirements when matching our clients with potential family model providers. We then facilitate meetings for our clients and host families to get to know each other to determine if they would like to move forward with the placement.
